What is Case Management?

Case Management is a collaborative process used in addiction and mental health treatment to ensure clients receive the services and support they need throughout their recovery journey. It typically involves assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s holistic health needs.

In substance use treatment, case managers help clients navigate healthcare systems, obtain housing, access financial aid, and engage with support services like therapy, job training, and legal assistance. They serve as a consistent point of contact, ensuring continuity of care and reducing barriers that could hinder recovery.

Case management is particularly critical for individuals with complex needs or co-occurring disorders. By building a network of support and maintaining open communication among service providers, case managers help reduce relapse risks, improve health outcomes, and promote long-term recovery and independence.

In legal or clinical contexts, case management may be mandated or offered as part of court diversion programs, Medicaid services, or community mental health initiatives. It is an essential component of comprehensive behavioral healthcare.
